Non-Immigrant Visa

Requirements for U.S. visa application forms:
  • Respond truthfully and completely to all the questions.
    All forms (DS-156, DS-157, DS-158) and the Questionnaire must be filled out completely.  If you have no response, put "none" or "nil.”
     
  • Provide correct contact and address information.
    Provide contacts both in Bangladesh and the United States.  Applications without specific contacts/addresses will be delayed until additional information is provided.
     
  • Sign your application, even if someone else fills it out.  If someone else fills out your application, they must include their name and contact information in the appropriate space.
     
  • Use your full name.  If your name on your passport uses initials, you must list your full name in the "other names used" box.

Important note:  Incomplete U.S. visa applications may result in delays in visa issuance, possibly of several months. All applicants are personally responsible and accountable for the answers on their application forms, regardless of who fills out the forms.
